Graduate Medical Education Program - Personnel Records

This series consists of personnel records maintained for graduate trainees and interns. The records span the period from 1968 through 1978, with the great bulk dating between 1970 and 1976.

Generally, each folder includes a completed application for graduate training; clinical training agreements (contracts) signed annually by trainees and representatives of the University; a temporary certificate of registration issued by the Illinois Department of Registration and Education; a personnel data form; income tax withholding certificates; letters of reference; and change of status notices recording actions such as terminations and changes in name or salary. Files for foreign students often include photocopies of diplomas, transcripts, and other credentials (with translations of those not in English), and sometimes contain correspondence with the U.S. Immigration and Naturalization Service. Occasional files include such documents as letters of reprimand, correspondence concerning grievances, change of address notices, and release forms for employment information requested by credit agencies.
